Home and Community-Based Services (HCBS) provide individualized, hands-on support for youth who are at risk due to medical or behavioral health needs. Services are delivered one-on-one in the youth’s natural environments—such as the home or community—and focus on meaningful engagement, supportive supervision, the development of life skills, and promoting independence and resilience. Referrals are made through the youth’s Care Manager as part of the New York State Children’s Waiver program, ensuring that services are coordinated within the youth’s overall plan of care. Suffolk Health delivers these services across New York, including Long Island, connecting families with the support they need close to home.
