At Suffolk Primary Health, we believe every child deserves the chance to thrive—no matter their medical, emotional, or behavioral challenges. Our mission is simple: to build stronger, healthier communities by empowering children and their families.

What Is HCBS?

HCBS stands for Home and Community-Based Services—a life-changing program under the NYS Department of Health. It’s designed to help kids and teens (ages 0–21) with medical or mental health challenges that put them at risk of hospitalization or out-of-home placement.

Our support is:

Individualized – Every child receives a customized service plan tailored to their unique needs, strengths, and goals.

Community-focused – Services are delivered in the environments where children feel most comfortable—at home, in school, and in their communities.

Growth-Oriented – We help children and youth build confidence, independence, and real-life skills through meaningful, hands-on learning experiences.

Family-Centered – We recognize that children are part of a larger support system. Services are available not only for struggling youth, but also for parents and caregivers—ensuring the entire family is supported and empowered on their journey.

Children and youth ages 0–21 who:

Are Medicaid eligible (or may qualify through other state criteria)

Have a serious emotional disturbance, developmental disability, or complex medical condition

Are at risk of hospitalization or out-of-home placement

Respite care provides short-term relief for caregivers and enriching, supportive experiences for youth.

Palliative Counseling & Support

This service provides compassionate counseling for individuals and families facing chronic or terminal illness, focusing on emotional well-being and navigating life’s challenges with dignity.

Offers practical tools for resilience, connection, and support

Prevocational Services

Prevocational services help youth (14+) explore careers and build foundational skills for future employment. Through career exploration, volunteer experiences, and workplace readiness training, participants gain confidence, soft skills, and real-world experience.
